Saline implant removal. Will they be smaller than the original, sag, or will they look about the same?

When implants are removed, the breast shape will depend upon many factors including: 1) How elastic is the skin and breast tissue, 2) weight loss or gain, and 3) pregnancy after the augmentation.  The breast shape is typically flatter and droopier than the pre-augmentation shape after the implants are removed.

Without the current pictures and the benefit of physical exam- it's difficult to comment accurately. Therefore, we don't know what kind of anatomical changes you had after 10 years of having implants. So, your post -explant condition will depend upon many factors. I would discuss it with your Surgeon during your in person exam.

HelloWith saline implants, a helpful technique can be to sterilely puncture and deflate the implants in the office before making a definitive plan for a next step. The breasts often look somewhat lumpy and odd right at first, but after several weeks, the appearance usually improves. This gives some women a better idea of what they might look like without implants. If you were happy with your appearance at that point, you could simply have the implant shells removed under local anesthesia. If you decided that you weren't happy with the result, you could make a plan with your surgeon for more surgery. Small breasted women who have moderate sized implants (like you) often do just fine with removal of their implants, as long as they have realistic expectations and can accept a somewhat "deflated" appearance.Best wishes- 

Independently of implants, your breasts are now ten years older compared to when you had your augmentation, so it is unrealistic to think that they would go back to their original look after removal. In fact, because of the implants, they may now look worse than they would have done if you didn't have your operation.It is unfortunately impossible to predict if you will need an uplift or not, but if you start with deflating your implants you will know by the time the complete removal will be planned.

I would recommend deflating your implants under local anesthesia. It will talk few weeks till your breast will go back to original shape and size. You can decide at that time to just remove your implants, get another set of implants or get fat grafting. Please see below link for patient with same issues. 

It can be difficult to determine how your breasts will look after implant removal. If you have gained weight since you had them done, you likely will be a bit larger than what you started out with. AS for sagging, they definitely will be deflated.

Thank you "Sakedrop" for adding your photos and new information with this re-post.There are several issues here.I now understand that you planning to remove them because they are hard.Usually, when they are hard, they are unattractive. I will assume that your saying that you have trouble breathing means that you have pain.Pain is not unusual in patients with hard capsules under the muscle. I see patients who have immediate relief of pain after implant & capsule removal from under the muscle.I have already answered the other part of your question which was posted previously. It would be a shame for you to think that you were not a patient who can live with soft enhanced augmented breasts. There are two options:You can remove them with or without removal of the capsule. You could then live with them for a while and decide later if you would like a modern implant placed which will give you a beautiful natural comfortable feeling.These decisions are best made in consultation. Hello. Removing saline implants is an option, and simple to do. The question is- what will your breasts look like ! You are lucky you had  moderate size implants, so your skin is not stretched out much. Also, the weight of the implant probably has not changed the shape of your breast tissue much... so I think it will look very similar to your pre-op pictures. It does take some time for your skin to shrink up, but is should. Good luck.

I would recommend total capsulectomy, antifungals around the time of surgery (trouble breathing is probably due to mold biotoxins which decrease VEGF (Vascular endothelial growth factor) and cause problems in many women), and an internal mastopexy in order to reshape the breast during surgery, deal with the extra skin and make sure you have a fold.  We have done thousands of explantations here and I wrote the book on the problems with breast implants.  Most plastic surgeons do not understand these problems and if they decide to puncture the implant during or before surgery, you can become very ill.  Most also do not understand the importance of total capsulectomy when you have systemic problems.
