The abdominoplasty incision interrupts both lymphatic and venus drainage causing swelling above the incision which may last a very long time.  The lower abdomen and upper thigh area below your abdominoplasty incision had less fat than your abdomen per se, therefore you still have not only swelling but residual abdominal fat which is bulging above your incision.If you were my patient, I would liposuction your abdomen as a second stage procedure which would eliminate the "step off" appearance.  At this point, I suggest you discuss possible options with your Board Certified plastic surgeon.

will cause a prolonged swelling time. You will simply need more time to heal from your surgery, realistically, 9-12 months is the time frame you should be looking at to heal completely after having a hematoma.

Everyone heals differently- after removal of hematoma, that will prolong swelling. Typically, 6-12 months are required. Some patients may also have a small amount of fat leftover which if does not go down on its own, will respond to liposuction.Close follow up with your plastic surgeon is recommended.

The tummy tuck operation interrupts some of the fluid drainage called lymphatics.  This could lead to increased swelling particularly in the inferior portion of the abdomen.  Furthermore your doctor should examine you for a seroma collection if he thinks you may have a fluid collection versus just swelling.  I perform liposuction on some patients after the tummy tuck operation particularly in the lower portion of the abdomen and in the flanks.  If this was not done you may want to consult a plastic surgeon on his thoughts of this procedure as well.
