I am interested in reducing my mild scleral show as well as my upper eyelid exposure. I have heard that upper eyelid filler can be effective for reducing upper eyelid exposure, but I am less certain about the best approach for scleral show. I have come across a wide range of recommendations, including canthoplasty, canthopexy, lower eyelid retraction surgery, and orbital implants. Given the variety of opinions, it has been difficult to determine which treatment options are most appropriate for my specific case.

Dear Garrett,

Your upper eyelids actually look fairly normal for a male face to me.

The white showing under your eyes appears more related to lower eyelid position than excess upper eyelid exposure.

The key question is whether this has always been present or happened after surgery or injury.

If natural and mild → I’d be conservative.
If caused by surgery → treatment may range from tightening the lower lid to more advanced reconstruction if tissue support is missing.

This is one of those situations where the correct diagnosis matters more than choosing a procedure.
