I'm 40, 5'2, weight shifts from 145 to 148, fair skin. Coolsculpting done x2 in 2021 under chin. Lipo done in 2022 under chin with min result. 2023 neck and lower face lift but not to the middle section of my neck that shows the biggest issue. The middle portion wasn't done in 2023 bc scarring from just the lipo really bothered me for awhile. Does it look like I need a revision of the lift with corset playtysmalplasty under the chin?

All of these are excellent reasons to undergo a revision facelift. The best way to treat the pixie ear deformity is to lift the deeper tissues and alleviate the tension on the ear lobule/face skin interface. I would also re-excise those scars behind the ear and re-close them without any tension....

Pixie ear deformity is due to excessive tension on the skin at the time of closure. The best way to fix this is a revision facelift. This will permit recruitment of the extra skin/tissue laxity by lifting and tightening the deeper tissues while alleviating the tension on the skin closure.
