There is a tail of cartilage that often extends into the earlobe. This cartilage can be used to control the earlobe position.  Removing skin will usually fail to correct 

With the Merck Stitch Method, earlobes can be brought closer to the head with a suture, simply and without cutting them open. The prerequisite for this is that the ear cartilage extends far enough into the earlobe. This can be clarified in advance by an examination and palpation of the earlobe. Alternatively, there is still the possibility of cutting open the earlobes and removing tissue. However, this is always accompanied by the formation of small scars.
