Unfortunately, there is no good surgical procedure to reliably and effectively change the position of the lower buttock crease.  Liposuction frequently makes it worse, irregular, or creates multiple creases, surgical buttock tuck results in an unsightly scar for a young 30 y.o. patient, and BBL is simply not effective for this. The loss of the crease at the lower end of the buttocks as seen in many may be corrected by a conservative liposuction of this areas.  When the fat is removed the bulge is lifted and the skin gets tighter.This area must be approached with skill as the crease can be lost requiring a direct excision of skin with its associated scar at the buttock thigh crease.Please consult in person with an experienced Board Certified Plastic Surgeon prior to making treatment decisions.

Often the sagging lower buttocks makes the buttock crease blunted, so the border between the buttock and thigh becomes blurry.  Buttocks can be lifted (or tucked) by incision on the upper part of the buttocks, making the buttocks rounder and fuller, but still the buttock crease tends to be inconspicuous.  In this situation, I often recommend to have a direct excision of the crease area, making the crease more visible and deeper.  This will create more distinctive border between the buttock cheeks and thighs.
