Wings or no wings? I was told by my surgeon that an implant with wings extending into the suculous would make my face boxy. Do you concur with this? We are planing on placing an anatomical in-plant. He is guessing a medium but will test during placement. I am also revising my neck and facelift at that time. Hoping for the jowls to move and to get a balanced looking face with chin projection.

Answer: What type of chin implant is best? A wraparound, anatomical chin implant with wings are the gold standard should implants today which also not only augment the chin forward, but give added width in the pre-jowl sulcus. The pre-jowl sulcus is the area just in front of the jowls which is present. A facelift can lift the jowls, while a chin implant gives a three-dimensional augmentation to the chin area. They do not make the chin look boxy. In our practice, we use Implantech solid silicone wraparound style chin implants, which can be custom carved when necessary. Answer: Told by my surgeon that an implant with wings extending into the suculous would make my face boxy. Hi, I have performed many facial shaping procedures, including Chin Augmentation with dermal fillers or silastic chin implants, for over 30 years. From the photos, your chin is quite weak. When the chin is weak, this creates an imbalance making the nose appear larger, the mid face top heavy, the lower face looks short, de-emphasizes the lips and allows early formation of a "double chin". Proper placement of a silastic chin implant adds forward projection to the chin thereby creating harmony and balance to the lower face. Using the same incision, liposuction can be performed to reduce the fat and further shape the neck. Excess skin, from below the chin, can also be removed through the same incision. I have found that placement of a silastic chin implant, through a small curved incision under the chin (also allows excess skin removal) to be very safe, quick, highly effective and far less invasive than a sliding genioplasty. I perform chin implant surgery in 30 minutes or less, often using a local anesthetic alone. In my opinion, you are a good candidate for chin implant surgery. In the 1970's and 80's chin implants didn't have wings and the implants made the chin look too forward giving the nickname "pharaoh chin". In the 90's anatomical and the even more natural appearing extended anatomical chin implants became available and in my experience and humble opinion they remain the "gold standard" in chin implants. Wings create a natural taper effect to the implant and will not give a boxy appearance, in fact it's just the opposite. An implant without wings creates an over-extended boxy look to he chin because the implant doesn't taper on the sides.Hope this helps.
