Thanks for your question! Buccal fat removal doesn't take 3-4 minutes. I don't know anyone who does it that fast and frankly- I don't think you'd want it done that fast. The procedure is relatively simple for a well trained facial surgeon but one that does carry risk. The incision must be made to avoid the stenson's duct which is the pipelline delivering saliva to the mouth from the parotid gland. The dissection can be tedious and must respect and not injure the buccal branch of the facial nerve. Lastly, symmetry is incredibly important so equal amounts of fat or close need to be removed from each side.
