What can be done about my lower face (chin, nose, long philtrum)? (Photo)

It appears in the photos that you have a short chin that is slightly forward. The deep mentolabial fold gives me the indication that the chin is short. Typically, the chin is lengthened and moved either forward or back with a sliding genioplasty. The surgery is performed in the office with intravenous sedation and you don't need braces. The length of the upper lip can be changed with an upper lip lift to show the front teeth at rest.

You would benefit from a rhinoplasty to refine the tip and increase your angle. You would also benefit from a lip lift. A lip lift does 4 things: shortens the distance between your nose and upper lip, increases the mucosa of the upper lip, increases the incisor show and pulls up the corners. You may also benefit from a chin implant to soften the projection of your chin.

It would be difficult to give you good advice based on these few photos. Full face front and profiles would help to see facial relationships and proportion. Nose, lip, possibly neck surgery should be considered? In person or teleconference consultation with a plastic surgeon with good reputations nose and face surgery is recommended. Good luck.

In general, when somebody has a small Central upper lip, along with a long philtrum, lip lift surgery tends to address both of these issues. You appear to have strong chin projection, and a chin implant would only make it stronger. If you're hanging columella is bothering you, you may wish to consider rhinoplasty surgery.
