Dear Roxy727,I understand your concern. However, without a proper assessment it would be difficult to determine what is wrong. Based on your photos, it looks like the problem might be in your incision scar. It is best that you visit your plastic surgeon for further assessment. Good afternoon!You do have a little spot where the scar tissue from your incision is stuck down to the muscle and capsule, creating that distortion when you flex. Try massage-rolling it between your fingers, etc., which can help reduce the distortion. Otherwise you can have a scar revision but of course that means surgery. Fortunately it seems focused under your incision and not throughout the entire breast, which is a much more difficult problem and one that is much more likely to result in needing surgery.

If it occurs only with animation of the muscle, it may be an area where the pectoralis major muscle and/or breast implant capsule are adherent to the undersurface of the skin scar on the areola. This can sometimes be fixed under local anesthesia with a scar revision, sometimes including the placement of fat, dermis or other grafted material to separate the skin envelope from the deeper, tethering structures. Sometimes it's not worth going through more surgery for a phenomenon that is only visible in certain movements but that is a personal judgment call. Exam in the office is required in order to assess what is going on and properly counsel you on options.
