I haven't had filler in 2 years. What are these lumps on my lips? (photos)

I've had 3 treatments of 1 mL of Juvéderm Ultra Plus within 5 years, and haven't gotten any in 2 years. I have this uneven "keyhole" in the middle of my lip, and a big lump at the bottom. What caused this and how can I fix it?
