The problem is not a skin related issue. The problem is based on having an uneven fat distribution from poorly done Liposuction. Your treatment was aggressive and your surgeon did not use proper technique. Unfortunately, there is no treatment for this type of outcome. Liposuction is a permanent and irreversible operation. If done, poorly results are permanent. The problem is not related to the dermis, but rather the tissue layer below the dermis or subcutaneous fat. Treatment options some providers attempt to correct poorly done. Liposuction include revision, liposuction, fat, transfer, or converting the procedure to a skin tightening operation. None of these options are great options in your case. Delivering consistent quality liposuction results is more difficult than most people believe. The number of providers who can consistently deliver quality results is relatively small. Liposuction results has more variation in outcome based on who does the procedure and any other plastic surgery operation. Patients should be highly selective of who they choose as their provider. I recognize that after the fact telling you how to pick a plastic surgery provider is not going to fix your situation. When choosing surgical procedures, it’s important to first have a clear understanding of what the underlying problem is, if you find it radio, frequency treatments and micro needling is helping then who am I to tell you not to try or use those treatments. They are however, not aimed at addressing the problem, which is that you have an abnormal and uneven fat distribution that was created by poorly done Liposuction that was too aggressive, and not done well. I would be very reluctant to have a secondary procedure, especially anybody who feels they can give you significant improvements with further liposuction. The same is true for using energy devices, including facetite, JPlasma, a.k.a., Renuvion or VASER all of these devices, have the potential for making your condition worse. If you opt to have surgical intervention, then you need to be absolutely certain you’re in the hands of the most talented, and experienced provider when it comes to liposuction and fat transfer procedures. Finding the best provider is no easy task. Generally it requires having numerous in person consultations at which time patients need to take an active role in vetting plastic surgeons regarding their skill and experience. This should include asking providers to open up their portfolio and show you their entire collection of before, and after pictures of previously done procedures. For Primary, Liposuction, and experience provider should have no difficulty showing you a minimum of 50 sets of before, and after pictures. Revision work is not nearly as common, and each revision case will be very unique. For this reason, it is not likely you’ll find any provider who has those kind of numbers when it comes to before and after pictures of revision surgery Generally, the best providers are doing revision work are generally those who are the best at the primary procedure. Whether a revision attempt is worthy or not is a difficult decision to make and then your situation is quite complex, and it will for sure require an in person consultation with an examination revision work can definitely make things worse especially if the wrong procedure is applied. Recognize that if too much fat was removed, any attempt at doing more of the same is more likely to make things worse rather than better. Grafting fat may be an option, but doing so successfully is technically difficult, and will inevitably require multiple procedures. This two has the potential of making the outcome worse. I’m sorry this happened to you. The truth is a substantial number of plastic surgeons simply aren’t very good at Liposuction. Best, Mats Hagstrom, MD
