It appears that your skin is very fine and small wrinkles like these show more than usual. Skin thickening/thightening with radiofrequency based devise and/or injection of nano fat grafts in that area may be something to consider. Hope this helps.

I would recommend a deep chemical peel. Sometimes this requires 2-3 chemical peels. This is an in office procedure done with topical and nerve blocks.

You do have some great options to reduce or soften the smile lines. Having filler done for mid-face is fantastic, but there is more you can do. (It is always difficult without being able to examine in person or very high quality photos; I also wonder if there is some mild jowling and fullness of the anterior cheek at rest). A non-surgical treatment for rejuvenation of the lower face and neck is now available called Embrace. It is a combination of treatments (FaceTite and Morpheus8) which are radiofrequency energy delivered to the skin and subcutaneous fat. As advertised, these procedures help tighten the skin and restore elasticity by encouraging the skin to produce more collagen and elastin. A combination approach would give the most improvement. Deep wrinkles and smile lines can be improved with a combination of Botox and/or fillers for the lines themselves and to build anatomical support with Sculptra for collagen building. Ultherapy is a great skin tightening device, but works best in combination with other treatments like J Plasma, RF treatments, lasers, microneedling/PRP, salt facials, Cosmelan for skin tightening and improve skin tone. This is a fairly common complaint seen frankly quite commonly in any active cosmetic dermatology practice.. The use of an appropriate low viscosity, malleable, cohesive HA filler properly injected below and perperndicular to these lines serves as a support and buttress to minimize their appearance significantly with smiling. In experienced hands, treatment should take no more than about fifteen minutes with immediate results. Threads won't do it and besides are expensive, have a high adverse reaction rate, which includes skin dimpling, divets, stitch extrusion and infections. Fillers for the upper face will generally not affect these lines at all. And expensive fraxel lasers and other energy-based devices are far more backed by social media hype and advertising than by any hard science. So, save yourself time, money and frustration by exercising a healthy dose of "buyer beware" if offered any of these. Hope this helps.

Hi there! Thanks for your question. That is a difficult area because a lot of tension is placed within skin as the muscles for smiling attach to the sides of the mouth at the commissure. I have done thinner fillers like Volbella, Kysse, and Redensity with some success. I do undersell the area because the forces when smiling tend to push the filler away from it's intended area. Softening can occur, but it's sometimes underwhelming for patients. Topical skin care in the area and peels may offer some help. But, I agree it's a tough area. Hope this helps!

Doing something with radio frequency and ablative will help. You will likely need 3 to 5 treatments but i think you would have great results but you will also need to be patient.
