Breast augmentation refers to placing an implant to increase the size of the breast, which is the opposite of your goal of masculization of your chest. There are a few options for FTM gender affirming procedures and the one that is used depends on your current anatomy and your goals for the procedure. Double incision, free nipple graft procedures are the most common that I perform for my FTM patients. In this procedure, the breast is removed and the nipple and areola, which are usually reduced in size to give a male appearance, are placed as a skin graft. Another option is a keyhole procedure in which the nipple and areola remain attached to underlying tissue for the blood supply, this can leave sensation in tact, but is limited to men who have minimal sagging. A periareolar procedure is performed through an incision just through the areola and are only used in very small breasts. Find a board certified plastic surgeon who has experience with gender affirming procedures, and consider getting more than one opinion as this is an important part of your transition and you need to be comfortable with your decision.
