Unfortunately, this is a poor photo that does not show much of the presented deformity. However, it does show that you have very deep tear troughs which will require a transconjunctival lower blepharoplasty. An incision is placed inside of the patient's eyelid, the orbicularis muscle, and tendon insertion (which creates TT deformity) is released from the bone attachment, and small local fat flaps are placed underneath to fill the groove. In some patients, additional fat grafting of the cheek/eyelid connection may create further improvement. Hope this helps.

Unfortunately, a full face photo clearly showing the cheeks would have been extremely helpful. Since the upper cheeks constitute a major support for the lower lid area, age-related loss of volume in the cheeks contributes greatly to the formation of the tear troughs. So, if volume in the cheeks is at the root cause, then the use of a robust HA volumizing filler can not only improve and restore cheek contour, but also secondarily improve the tear trough issue. If still necessary, a low viscosity, malleable, cohesive HA filler can then be used to further elevate and smooth the tear troughs. In experienced hands, both of the aorementioned can be performed in about twenty minutes with immediate, on-the-spot results. Hope this helps. Best of luck.
