I have gotten fillers in my cheekbones in the past year, but because of age, I am noticing increased loss of electricity, causing deeper pockets in my under eye, like U-shaped wells. I have dark purple discoloration under my eyes. Is there any treatment that would smooth out this area and increase the fat so the purple isn't as noticeable? Please and thank you.!
Answer: Undereye Filler and/or PICO laser Hello! My name is Jessica, I am a physician assistant, and one of the providers at Aesthetic Skin - Winnetka & Chicago. This is a great question. For treating under-eye hollows and dark discoloration, a combination of undereye filler and Pico laser can be an effective approach, as each addresses different aspects of the problem. Undereye filler, typically composed of hyaluronic acid, is ideal for restoring lost volume in the tear troughs, which can help reduce the shadowing that contributes to the hollowed appearance. By gently lifting this area, fillers create a more youthful, refreshed look without adding puffiness or bulk. For dark discoloration, which is often caused by factors like thin skin, blood vessels showing through, or pigmentation, Pico laser can be an excellent complement. The Pico laser uses ultra-fast pulses of energy to target pigment and stimulate collagen production. It works by breaking down the excess melanin responsible for the dark circles and improving skin texture, which can make the area appear brighter and more even-toned. Combining these treatments can provide both immediate volumizing effects from the filler and long-term skin improvement from the Pico laser, resulting in a smoother, more rejuvenated under-eye area. I hope this helps and best of luck!

Answer: Effective Treatments for Under-Eye Discoloration and Volume Loss This is a common issue that can be addressed with several treatment options to help improve the appearance of the area. Dermal Fillers: Hyaluronic acid fillers, such as Restylane or Juvederm, can be used to fill in the tear trough area, reducing the appearance of dark circles by smoothing out the contours and minimizing shadowing. This can help create a more youthful and refreshed appearance. Fat Transfer: For a more permanent solution, some patients opt for fat transfer, where fat is harvested from another part of your body and injected into the under-eye area. This can help restore volume and improve skin texture. Laser Treatments: Non-invasive laser treatments, such as fractional lasers or IPL, can help improve skin texture and pigmentation by stimulating collagen production and reducing discoloration. Topical Treatments: Using eye creams that contain ingredients like vitamin C, retinol, or peptides can help improve skin elasticity and reduce pigmentation over time. Lifestyle Adjustments: Ensure you are getting adequate sleep, staying hydrated, and protecting your skin from sun exposure, as these factors can also contribute to under-eye discoloration. I recommend consulting with a board-certified dermatologist or oculoplastic surgeon who can evaluate your specific situation and recommend the best course of action tailored to your needs.

Answer: RE: discoloration under my eyes (photo) Hello, this is Dr. Hong from Fresh Clinic in Seoul, South Korea. Based on your photo, I can suggest a few possible reasons for the discoloration and puffiness under your eyes: 1. Fat pad displacement under the eyes, which can create shadows and make the area appear darker.2. Filler migration, which occasionally happens and can cause a bluish tint under the eyes.3. Genetic vascular factors — individuals with thinner skin and more prominent blood vessels (especially among Caucasians and Europeans) often appear to have darker under-eye circles. If my assumptions are correct, I would recommend considering under-eye fat repositioning as the first step. This procedure helps smooth out the contour under the eyes and can visually reduce the color contrast between the under-eye area and the rest of the face. If filler migration is contributing to the issue, dissolving the filler may also be necessary beforehand. I hope this explanation is helpful! Warm regards,Dr. Hong

Answer: U-shaped wells under my eyes Discoloration under the eyes is typically caused by volume loss and can be corrected with filler injections of hyaluronic acid in an area called treat tough. Filler injections in the area are complicated and there should be consideration placed on skin tightening as the years go by with modalities like IPL and Lasers.

Answer: Dark purple discoloration under my eyes (photo) Hello Natalie, If you are ready for surgical correction then I would recommend an expert oculo-plastic surgeon to perform a fat transposition lower blepharoplasty where your lower orbital fat is relocated into the hollow undereye area. If you are not yet ready for surgery then I would treat you with EZ Gel prf (platelet rich fibrin). In either case it is most important that you choose an expert physician. Feel free to contact me to schedule either a virtual or in person consultation to address any further questions or concerns. Good luck and be well. Michael A. Persky, MD
