Hello and thank for your time I had gland/fat removal 3 years ago and on the left side too much was removed leaving me with this result. I have been back and forth with the company/surgeon over the last 3 years have seen seen 3 different surgeons all of which have said I need to exercise my pectoral muscles and build my chest up to fix this as nothing else will? This is very difficult to fix and fat grafting will take many sessions with limit success. Whats the truth? Am i stuck like this now?

Thank you for sharing your question and I am sorry that your result has not been what you anticipated.  The depression created was due to overly aggressive removal of underlying tissue from the areola, but it can be improved with fat grafting or local fat flap rearrangements, as well as some suturing techniques to help support the tissues further.  I would see a gynecomastia specialist in consultation to discuss the best options for you. You have a crater due to too much removal of tissue directly under the areolas.  The treatment would be a fat flap procedure, which will move tissue from adjacent areas into the crater to fill it out.  I have done hundreds of these revision procedures.Elliot Jacobs, MD, FACSNew York City
