It's hard to tell from your photos, but it looks like the surgeon probably took out too much fat in that part of your arm. I would consider trying morpheus (radiofrequency microneedling), it's simple and might help tighten the skin a little and help you avoid more surgery.

Hello; After excessive implementation of Renuvion J Plasma technique, in the subcutaneous tissues and skin; due to excessive fibrosis, wrinkling of the skin and when the arm is lifted, band formation; that creates for tension; may occur. In general, that problem; starts to decrease after 3 months period. It continues for decreasing until the 6th month as well. Unfortunately, steroid injection; causes for depressions.
