Which procedure would reduce under eye bunching when smiling? (Photos)

Question

I am 20 years old, and a female. When my face is resting, I have dark under eyes, and minor eye. I have a fair share of other flaws, but all in all, I can tolerate them (although my under eyes bags without makeup are still terrible so I’d like to find a procedure for this too). However, the moment I smile, my face is ruined by significant under eye bunching and folding. I notice that this bunching also occurs when I am talking/ making facial expressions (but it’s less significant).

When you smile, you appear to have a large orbicularis muscle that forms a roll on both sides, and it appears to be larger on the left side than the right. This is also associated with small fat pads and you also have crow's feet when you smile. Perhaps you may be a candidate for lower eyelid blepharoplasty depending on your examination.

Crow's feet at the lateral corners of your eyes would be well addressed with a neurotoxin like botox. In our experience, injecting the orbicularis roll with botox may not lead to significant Improvement. Fillers may lead to a more puffy under eye appearance so please be cautious. I hope this helps! You are beautiful! But Botox will help to soften the bunching around your eyes when you smile. We should all have some movement/scrunching around our eyes which we refer to as "crows feet". A little movement is needed as we express ourselves with our eyes and our smile. But we can soften the amount of bunching that occurs with a little bit of Botox. Hope this helps!

This kind of "bunching" typically results from an overly active under-eye muscle. The use of a couple of microdroplets of Botox injected into the belly of this muscle can diminish this considerably.  Volume deficit in the adjacent upper cheeks typically accentuates this problem. So, if this is indeed the case here, the use of an HA volumizing filler for the cheeks will not only create a more desirable oval, heart-shaped face, but will secondarily further improve the under-eye regions. You would be wise to consult with a board certified cosmetic dermatologist and best of luck.

Botox can be used to improve wrinkles around the eye. Deeper wrinkles need a combination approach with Botox and lasers, microneedling/prp and fillers. I recommend getting a formal evaluation with a cosmetic dermatologist. Best, Dr. Emer
