I had bad liposuction to the upper back of my thighs, right below my butt at my gluteal fold. Most of the fat was removed from this area and it is now quite hollow (instead of having fat). Because there is no support, my butt has also dropped down to rest on the top of my thighs. I have had a fat transfer but the fat did not take. Is it possible to inject quite a lot of hyaluronic acid filler and/or Sculptra to the area to help restore the missing volume underneath my butt and provide support?
Answer: Can I use filler for to restore this volume? From the photos and description of your concerns Sculptra or HA fillers would not be an economical choice to restore your volume. Too much product would be needed and the results would not be permanent.

Answer: Shipping options for poorly done Liposuction The fillers are not a realistic option unless you’re independently wealthy. HJ fillers are typically around $600 per milliliter and most likely several hundred milliliters of fat was removed during your procedure. Even spending a few thousand dollars will be insufficient and on top of that the results will be temporary. Treating previously over treated liposuction areas with that transfer is possible but the inherent problem is that the area that needs volume most has now been devoid of host tissue making retention or survival of the fat graft small. in order to get success the procedure needs to be done multiple times grafting a conservative amount each time gradually building up the subcutaneous fat layer until it’s sufficient. trying to do it in one procedure leads to an increase loss of grafted fat with more fat necrosis and excess scar tissue formation. it’s a mistake to try to do this in one session. It needs to be a staged procedure for the surgeon and gradually builds up the volume. if done correctly that an appropriately removed fat can be restored. other than exciting the area and attempting a reconstructive approach with an advancement or rotation flap there is really no other option. Treatment solutions should be based on a clear understanding of what the primary or underlying problem is. In your case it’s a lack of subcutaneous fat from inappropriate liposuction. I hope you at least followed up with the surgeon who did the primary procedure and let him or her know what the results look like and that you’re not happy with it so that that provider knows not to do this again. you may also want to consider writing a professional and appropriate review regarding your experience. I’m sure other patient interested in Liposuction from this provider would benefit from hearing your experience. Delivering consistent quality liposuction and fat transfer results is far more difficult than most people believe and this includes plastic surgeons. Results are often final and attempting to correct previous poorly done work is difficult and sometimes not possible. Considering that revision work is many times more difficult than primary liposuction or fat transfer patients need to be even more selective about finding the most experienced and talented provider. Do that by having multiple in person consultations for local board-certified plastic surgeons in your community. I recommend avoiding virtual consultations whenever possible. During each consultation ask each provider to show their entire collection of before and after pictures for the procedure you’re interested in. Simply seeing a handful of pre-selected pictures which most likely represent the best results of the providers career is insufficient to get a clear understanding of what average results look like in the hands of each provider. this is why patients need to take it upon themselves to properly vet providers by asking them to open their portfolio and show their entire collection or at least as many as they’re willing to. That’s the provider to show as many pictures of patients with similar body characteristics for your own. You may also want to ask to see examples of excellent outcomes, average outcomes in less than outcomes. An experience provider should have no difficulty showing you at least 50 sets of before and after pictures of commonly performed procedures. highly experience surgeons will have hundreds or even thousands of before and after pictures for commonly performed procedures. Revision work is not nearly as common as primary procedures and it’s not reasonable to expect plastic surgeons to have nearly as many examples of the results of revision work. still it is important and patients need some way to vet surgeons in regarding to skill and experience for the proceeder they’re interested in. I recommend continuing to have consultations with different providers until you feel quite comfortable you found the right provider to work with. With revision work some patients may not be willing to go through what it takes to properly correct the disfigurement. in your case it may take four or more rounds of fat transfer to get the volume restored correctly. Attempted revision work can also easily make things worse. It is critical to be working with the right provider and this is especially true for revision work. Finally I also recommend reading all reviews on various position review websites avoiding providers for any significant number of justified negative reviews. The absence of justified negative reviews is more important and more telling than having an abundance of positive reviews. I don’t think fillers or sculptra are going to do the job in your case. The banana roll is a skin fold in that area should never be treated with Liposuction. Doing so never improves the contour and can lead to direct disfigurement. It is not an uncommon mistake we see on a fairly regular basis. The key and most important valuable is finding the right provider to be working with. I’ve given you some suggestions on how to do that. There are unfortunately no shortcuts and vetting plastic surgeons takes time and effort. Best, Mats Hagstrom MD
