6 months post op from revision surgery. Original surgery was 360 liposuction & was left with saggy skin underneath breasts. Surgeon advised he’d do a reverse tummy tuck and remove my fibrosis. He advised the scars would be hidden under my breasts. However I am now left with a pocket of skin inbetween my breasts, which I do not want to cut. I have also been left with further fibrosis and severe lipo burn scarring. Scars under breasts not healing well either. Please advise on my options.Thanks

Thank you for sharing your history and photos. A reverse tummy tuck is a technically challenging procedure that requires meticulous preoperative planning and precise surgical execution. To avoid excess skin between the breasts, the upper abdominal skin must be carefully lifted and pulled laterally during surgery. Unfortunately, when this step isn’t performed, residual skin between the breasts, like what you are experiencing, can occur. The only definitive way to remove the excess skin between your breasts is through surgical excision, which would result in a scar between your breasts. Since you prefer not to have additional scarring, living with the skin may be your only alternative. For the fibrosis and lipo burn scarring, I recommend lymphatic massage to reduce inflammation and improve tissue mobility. Additionally, for the scars under your breasts, consider treatments such as steroid injections or 5FU injections to soften and flatten the scar tissue. Laser treatments like Fraxel can also improve the texture and appearance of your scars over time. I hope this helps provide clarity about your options. Answer: Plastic surgery outcomes I’m sorry for What happened to you. Your provider selection process was not done well. Was this work done in your own country by a board certified plastic surgeon? The procedures you had are permanent and a reversible. Unfortunately, if done poorly people can be left disfigured. At this point it’s going to be very difficult to improve your outcome in any way. Small partial improvements may be possible but in the big picture revision surgery may no longer be worth it. I cannot emphasize enough, the importance of provider selection, and properly vetting providers before selecting surgery. Delivering consistent quality liposuction results is more difficult than most people realize. I suggest patients do that by first having multiple in person consultations with board-certified plastic surgeons in their own community. During each consultation, ask each provider to open up their portfolio and show you their entire collection of before, and after pictures of previous patients who had similar body characteristics to your own. Being shown a handful of pre-selected images representing only the best results of a providers career may be insufficient to get a clear understanding of what average results look like in the hands of each provider. An experienced plastic surgeon should have no difficulty showing the before and after pictures of at least 50 previous patients. Truly experience used providers should have access to 100 or preferably thousands of before, and after pictures to choose from There’s no correct number of consultations needed to find the right provider. Considering how much variation there is a liposuction skill I recommend patients have at least four or five consultations before considering selecting a provider. The biggest mistake patients make is having only one consultation, then scheduling surgery. The second biggest mistake patients make is assuming that being board-certified and plastic surgery with a few years of experience and a handful of positive reviews is sufficient to confirm that they’ve mastered most plastic surgical procedures.
