I'm 35 and feel as though my face lacks structure and definition in the cheek, mid face and jaw area. I have had dermal fillers but wonder if surgery would be a better option? `

Hello, hope you are well. Facial structure is predominantly determined by the facial skeleton (cheek and jaw bones) and is concealed by overlying soft tissue. Fillers behave like liquid because that's what they are, and you can't sculpt a face very well with liquid. Facial implants behave like solids because that's what they are, and work much better for enhancing facial structure and definition in the areas you mentioned. Buccal fat extraction sometimes can work to enhance cheek and jaw definition, but it's impossible to say in your specific case without a photo of your face. There can be a fine line between appearing structured/defined and gaunt/skeletonized from aggressive buccal fat excision so the choice is individualized and should be guided by a facial contouring surgeon.
