Is it possible with surgery that I could achieve a round perky look? Best steps to take? (Photos)

Hi, i had surgery around 5-6 years ago. I had lost a lot of weight and left and decided to go with a lift and implants - i had a breast lift and implants (mentor moderate plus) around 425 cc pretty quickly I noticed more of a long enlogaged boob shape rather then round and perky. I believe that I Possibly have capsular also as one feel hard compared to the other. Is it possible with surgery I could achieve a round perky look? I don’t like how high up they sit on my chest - but they was placed under the muscle . I’ve Been advised by a doctor to have removal & lift and around 400 cc moderate plus profile texture placed but im just dreading round two.. do I seek more options ? what are your thoughts

To create a more perky look, I would do the following:
-Replace your current implants with mid-400's High Profile (NOT moderate profile)
-Revise your breast lift
P-lace an internal resorbable graft (Internal Bra)

Currently, your implants are sitting high and your breast tissue is effectively waterfalling over them.  To address this, I would definitely not use a Moderate profile implant but instead a High profile implant.  The Moderate profile implants are wider and flatter and fail to give nice breast projection because width is emphasized over projection.

From the stretch marks it appears that your tissue was stretched a good amount and so internal support (in the form of an Internal Bra) is necessary to hold the implants in place and position them more optimally.

The lift definitely needs to be redone since it is apparent that there has been tissue stretch after your original surgery.

I would definitely go with a Board Certified Plastic Surgeon who specializes in secondary breast surgery.  This is a little more challenging case and so you need someone who has a good amount of experience in this area.
