Botox has created a dent in the middle of my head. What is the best way to rectify it? (Photo)

I had my first botox done 3 months ago. It created a dent in the middle of my head. The doctor said the dent is just due to my head shape and there isnt much he can do about it. (He also suggested NOT to get filler) I never had the dent before botox. I am more insecure now than i was before i had botox because the dent is really visible in certain lights and looks horrible. Any suggestions?

Thank you for your question. The appearance of a dent or indentation after Botox, especially if it wasn't present beforehand, may be due to uneven muscle activity or atrophy in the treated area. This can happen when some areas of the frontalis (forehead) muscle are more relaxed than others, causing differential tension across the forehead and the appearance of a visible depression in certain lighting or angles.

This type of dent typically resolves over time as the effects of Botox wear off—generally within 3 to 4 months. Since you're now about 3 months post-treatment, the muscle activity should gradually return, and the appearance may improve naturally without additional intervention.

While your provider advised against filler—which can be appropriate in some cases—it’s also important to avoid overcorrecting in such a delicate and dynamic area.

For future treatments, precise adjustment of Botox placement and dosage can help prevent this from recurring. Be sure to share your experience with any future injector so they can customize your treatment accordingly.

If the dent does persist even after Botox has completely worn off, other treatment options such as microneedling, PRP, or subcision may be considered for skin texture or volume concerns, depending on evaluation.

The depressed area that concerns you is high risk for fillers.  You can leave it alone and it may improve over time or you can consider treatments such as SkinPen to help improve overall skin tone and texture.  Typically 3-6 treatments are recommended for optimal results.

I have been injecting Botox for cosmetic reasons all over the face and neck since 1991 and have yet to encounter a single case of indentation in the forehead (or anywhere else, for that matter) since then. 
On the other hand I have successfully treated steroid atrophy indentations of the forehead that appear quite similar to the indentation seen in the photo using an appropriate low viscosity, malleable, HA filler to immediately elevate and smooth it. You would be wise to make sure to consult a board certified cosmetic dermatologist filler expert and best of luck.

Whilst such treatments may 'unmask' underlying features of the forehead (e.g., muscle characteristics), the Botox shall 'not' have 'created' a dent in your forehead. I suspect that it is more likely that you had previously not noticed any contour abnormalities of your forehead (and are more critical after the Botox treatment). Take some photos and repeat the same in three months' time (in the same location with the same lighting), and then review at that point.
