Had reconstructive revision rhinoplasty last December with rib graft and then a small fat transfer procedure to the bridge of my nose recently. The revision left me with a retracted columella and small dent to my nose as well as uneven nostrils. Don’t want any further cartilage grafts but is it possible to do an alarplasty to bring nostrils up and in as well as skin/fat graft to columella to bring it down and improve scar as well as fat graft to fill in dent? Yes, I think you are a very good candidate for an alarplasty to reduce your bilateral flaring. In your case, you do not need any of the nasal floor excised, but just the alar walls!

It appears that too much alar wall was removed from your left nostril, which shortened and straightened it, with a concomitant loss of curvature. It would seem that you would benefit from a composite (skin + cartilage) ear graft to lengthen it and restore the curvature as it attaches to the face.
