What surgery should I get to lower my hairline, and feminize my forehead and make it smaller? (Photos)

I am a MTF pre trans, and I have a naturally very large forehead which I would like to make smaller, and I am also experiencing male pattern baldness on top of this. I want to get a surgery done to improve this, however I don’t know which would be the best option for me. I consulted a hair transplant doctor who advised that the hairline I desired is unachievable due to the muscles in the face. Which surgery should I get? Also would my hormone replacement therapy affect the hair growth?

Hormone therapy will both stop further male pattern hair loss and, with a more stabile hairline, allow for a surgical hairline advancement.  Otherwise, a hair transplant is your best option.  If your hair transplant surgeon told you that the desired hairline is not achievable, I would be cautious about shopping around for one that says they can do it.  They may be less experienced, overly optimistic, or not understand what you are looking for.  An honest and experienced hair transplant surgeon will rarely do a procedure for a young patient who is having continued hair loss - this can lead to problems in ten or twenty years. -Dr. Rolfes

In many transgender patients there can be hair loss due to taking exogenous hormones. Some trans women may want to change their hairline to make it appear more feminine. There are many different treatments that work for hair restoration, for example oral or topical medications and injections of PRP or steroids. Recovery from hair transplant is about 5 days, though most patients feel fine after 2 days. This procedure is great for male pattern baldness, thinning hair, and trans-masculine patients who lose hair due to taking testosterone. It is also effective for any patient who wants to change their natural hairline.

Thanks for your question and photos.  Without the benefit of personal examination and measurements I would not be certain, but you appear to be very suitable for hairline lowering foreheadplasty.  I would recommend that you discuss starting feminising hormone therapy and also finasteride or duasteride (dht blockers) before you have surgery, to improve your hair growth and soften your facial features.  Hope that this is helpful and best of luck.
