As long as the aesthetician goes over the areas with active acne last, you should not have a problem with the rest of your skin breaking out. SkinPen will help with pore size if that's an issue, but topical skin care products will also be great in combination to help with pore size and acne breakouts.

I would recommend not doing microneedling over any active acne, even small pustules. You may want to look into doing some salicylic peels to kill any bacteria and reduce inflammation.

Yes, this may even help but the aftercare would be key in preventing a breakout.  You want to avoid thicher occlusive emolients such as Aquaphor which might clog pores post treatement.  I would suggest PRP to promote healing and Alastin Nector post treatment.
