How do I get rid of loose skin and fat/soft tissue around jawline and chin?

Had a bite problem due to my mother's umbilical chord wrapped around my neck during birth. Had to get jaw surgery. Now I have loose skin around my jaw and chin. I had very tight skin before (pic below). Now I have a lot of loose skin around my jawline and chin and also a lot of fat/soft tissue (don't know what it is). How do I get rid of this *permanently* and get a tight face? Do I need to tighten my skin or do I need to get implants? The doctor had to cut/shave lot of bone to fix my bite.
