I would love to get fillers to help my face have some volume but my plastic surgeon is concerned about my history of dermagraphism and my anxiety about procedures going wrong. My self esteem is poor as most people assume I'm in my 40s due to my volume loss and smile lines. I'm very worried about blindness with fillers and not sure the risk, while low is worth it? I am already getting Botox in my forehead.

No one needs any of these aesthetic procedures! Some alternatives for volume loss are microneedling and Ultherapy which are known as CIT (collagen induction therapy) treatments. The SkinPen is a great microneedling device that helps improve fine lines and wrinkles and requires little to no downtime. Ultherapy uses ultrasound technology to deliver focused energy deep into the dermis which allows the body's soft tissue to contract and is the only FDA approved device to lift and tighten. Injections in the midface and lower face anatomically will not result in blindness, only treatment around the eyes carries this as a potential issue. Since you have lost overall volume, you may want to consider Sculptra or your own fat. Scultpra is semi-permanent and great for overall volumization. I don't often recommend it, but you are the perfect candidate. Autologous fat grafting is also a great, permanent option but would involve a surgical procedure.
