I find this implant not that useful. Made originally for the reconstructed breast and women who want a very artificial appearing breast. Many women I have seen have had these exchanged.
These implants are rarely needed except for reconstruction. Your surgeon no doubt felt they would not achieve your goals and so did not wish to cloud the consent process with irrelevant information. In general the result is equal to what breast tissue you have plus the implant. If you have reasonable breast projection then an ultra high implant may look fake or result in excess pressure on your breast tissue when can lead to thinning out of the breast and subsequent problems in revisional surgery in the future. Typically, a surgeon will choose whatever implant is the proper dimensions to achieve the end result that patient desires. There is nothing good or bad about one profile versus another, it is just which implant will best meet the goals in that individual patient. Probably, your surgeon did not feel ultra-high was the best choice to give you the end result look you desired.
In the right patients, I have found the Allergan 45 ultra high profile silicone implants to yield excellent results. There are several factors that plastic surgeons consider when recommending implant sizes. I consider two factors when selecting implants for my patients:1. Dimensional planning2. Gel Implant Sizing systemDimensional planning – The measurements of your chest wall are taken. Also, the breast dimensions including the height, width, and current dimensions of each breast form the basis of dimensional planning. Based on these measurements, the implant size is recommended. This will give you a unique breast implant that is suited for your body frame. However, there are some limitations of what size we can recommend. For instance, some implants may just be too big for a narrow chest wall. Your surgeon can review this with you during the consultation.Gel Implant Sizing system – During the preliminary breast implant consultation, you will be provided with an option to “try on” a variety of implant shapes and sizes. You can also visualize the possible outcomes of your surgery which helps you to get that perfect size to give you the shape that you longed for. Your surgeon may not have felt that the ultra high implant was in your best interest. This implant is typically reserved for reconstruction and very small framed women. It does apply more pressure on your own tissue than other lower profile implants, and long term, may not be the best choice.
Communication is key with your surgeon. If you are concerned about the information discussed before your surgery, you should ask your surgeon this question directly. Generally speaking, ultrahigh profile breast implants ( that have a very narrow diameter and relatively significant projection parameters) are useful for very few patients. These patients tend to be small framed and are looking for relatively “round, fake” outcomes. For most patients, in my practice, high profile or moderate profile plus, breast implants tend to meet patients objectives best. Of course, your plastic surgeon will be your best resource when it comes to receiving more precise/relevant answers to questions. I hope this helps.
Based on training and experience, different plastic surgeons will have different preferences for implant profiles. Higher profile implants have more volume for a given diameter and this applies to both saline and silicone gel implants. It is my opinion that high profile saline implants cause more long term breast tissue thinning and I have stopped using them. My implant of choice is a high profile gel implant for most of my patients as this gives a very natural looking result. The ultra high profile implants tend to create a rounder, slightly less natural looking breast. Hope this helps, good luck!
Thank you for your question. You need to discuss your surgeons choice of implant directly with your surgeon. Personally I have never used alter high profile implants but my understanding is that they are most often used for breast reconstruction.
