I had chin/neck lipo over a year ago,irregularities began to form very quickly after my procedure. What surgery is the best option to correct it? There is a line from what I believe is from the cannula itself and a pocket of fat that was missed. Thanks

A full set of facial and neck, photographs, and your age are gonna be required to make a determination about how best to proceed. Depending upon your anatomy, you may have Fat deposits located below the plasma muscle, above the platysma muscle, a short chin, hypertrophic muscles in the neck, and loose skin.

As a dermatologist, I see this type of concern after chin/neck liposuction more often than patients expect—and the good news is that it can be improved.

From your description, there are likely two separate issues:

- The “line” is very consistent with a cannula track and fibrous scar adhesion
- The fullness/pocket is likely residual fat that wasn’t fully contoured

At one year out, this is no longer swelling—it’s structural, so treatment needs to address that.

In terms of correction, the most effective options are:

- Minor irregularities: Sometimes improved with subcision (to release the scar) combined with a small amount of filler or fat grafting to smooth the depression
- Residual fat pocket: May require targeted revision liposuction, but this has to be done very conservatively because the area is already scarred
- More significant contour issues: Often best treated with an open submental revision (submentoplasty) where the surgeon can directly release scar tissue and precisely contour the fat

Non-surgical treatments alone are usually limited at this stage.

The key point is that this is a revision case, not a primary one—so choosing someone experienced in secondary neck contouring is critical. A careful, conservative approach typically gives the most natural improvement.
