My nose is slanted and split at the tip? What can I do about it? (Photo)

erag
I am so insecure about my nose. I've looked at pictures of myself when I was younger, and it did not look like this. I don't know what happened, but I hate it. Not only is it slanted/crooked, but it also splits at the tip (like a butt-chin, except on my nose). Why is this happening and how can I fix it? My teeth are also pretty badly slanted and I hate smiling for that reason.
