Why won't my 5 year old filler lump dissolve? (Photo)

Question

29 yo female who had 1.5 syringes of juvederm injected into lips in 2018. Lump was visible from the very first day of injection. I was told to massage it and that eventually it would go away. Between 2021-2022 I had six separate hylenex injections. My doctor said it probably wont improve any more. I do have severe allergies and imo a wonky immune system, not sure if that could contribute. Can I laser or excise the lump? Thank you!!

Hello, and thank you for your question. If you have had multiple rounds of hyaluronidase and it has not dissolved, it may not be filler and may be a Granuloma or nodule. The treatment for this will differ from hyaluronidase injections and may require steroid injections or excision. Additionally, your immune system could potentially affect the treatment course, but most likely not in this case. I would suggest consulting with a plastic surgeon who could better direct your course of treatment. Hope this helps!

This may be result of the scar around filler. Topical injection of corticosteroid may be considered. Otherwise, excision is a good option. You have to decide whether such a small deformity is worth of surgery?

Your lump is subtle, is it worse when you smile ? They may have to inject very superficially to get it to go down but unless it’s very obvious when you are smiling, the way you lips appear in the photograph lead me to believe that it is wiser to leave it alone and be happy. 99% of women would envy your lips.

Juvederm does not last 5 years and what was injected is now gone. Please take photos with your lips completely relaxed. Your muscle movement will make lips bunch up and look more lumpy.

Based on the submitted photos the lump that concerns you is very subtle.  You could consider excision, but that would be replacing one blemish with another.  It may be best to leave it alone.

I am not sure as to what lump you’re referring to because your lips appear to be absolutely normal. Before you put blame on your doctor, I would compare the before, and the after photographs, or at least with the lips that you currently have, and the photographs of you prior to the treatment.
