I’m 40 yrs old and I’m unhappy with the increasing heaviness in the bottom of my face, under eye hollows, flat cheeks and lumpiness around my mouth. I look mad all the time. I’ve had cheek filler done to my midrange and cheekbones, a little nasolabial filler and under eye filler(that totally disappeared). I’m open to whatever would best address my concerns but I’m not sure if a cheek lift would make enough of a difference.

I'm sorry that you were disappointed with the prior treatment(s) with filler. But, in my experience, failure to sufficiently improve the appearance (or relatively short-lived results) of a person of your young age suggests to me an inexperienced injector, or improper injection technique, or the wrong selection of fillers to get the job done. For example, many inexperienced, novice injectors typically rely on plain Juvederm or simple Restylane for all purposes. And, this is unfortunate, since each company makes robust fillers intended to be used for different problem areas of the face. For example, there are robust volumizing fillers for non-surgical cheek augmentation. If lift is required there are HA lifting fillers. If greater contour is required, say for the jawline and jowls, there are HA contouring fillers. And there are even low viscosity, cohesive malleable HA fillers for use in areas such a as under the eyes for bags and crinkles. 
Before jumping, especially at your young age, into expensive, aggressive, invasive surgeries that risk permanent scars, etc., you might consider consulting with an experienced, board certified cosmetic dermatolgist filler expert. Hope this helps and best of luck.
