Thank you for your question. A lower blepharoplasty is helpful for reducing eyelid fat pad show. Some surgeons such as myself also will transfer the fat pad into the tear trough to act like a natural filler that's permanent. You are a great candidate for lower lid blepharoplasty with fat transposition. This surgery utilizes an incision from behind the eyelid and moves fat from your lid to tear trough to smooth the transition from the lid to cheek. This essentially removes the puffiness from the lid.
If by prior filler was meant direct treatment of the tear trough region with a low viscosity filler, lack of success would not be surprising. I wonder if the more appropriate injection of a robust HA filler in the upper cheek regions (which are a very important supports and buttresses for the infraorbital region) was tried instead in order to help to improve the under eyes. If this was not done, then this would be worth discussing with your injector before jumping to aggressive, invasive, expensive surgery or the use of fat injections. Make sure that you consult with a board certified cosmetic dermatologist.

Based on this photo, you seem to be good candiate for transconjuctival lower blepharoplasty. Surgery is performed entirely through a small incision inside the lower eyelid and, the most significant signs like tear trough and fat bags above. are corrected. no need for outside scar in your case.
