65year old with capsular contracture. First augmentation in my late twenties. I then had revision surgery in my early 30’s for cc mainly on the left side. So implants are 30 years old and I have fairly significant CC again. I’m considering another revision surgery (vs. removal only) but would implant with a smaller implant under the muscle. Is using strattice recommended and are drains neccessay. I’m getting mixed opinions.

Saline implants are designed to be overfilled, and most of us do to avoid decrease the risk of ripples. You now have larger implants and your tissue may need to stretch some to accommodate them. Once settled, your nipples should be in good position if they were before surgery. Please discuss...
Your lateral pockets can be tightened to decrease side movement. Even with larger implants, you will still have a gap since this is your anatomy. I would not go larger, however, because you have thin tissue. Larger will look more like balls and tend to sag and push more to the sides.

the risk of lymphoma is very low. If your implants are not recalled, then the risk is even lower. I would suggest discussing your concerns with your plastic surgeon and making a decision.
