For someone who has just lost 100 pounds, fat grafting is likely to have relatively poor survival and therefore disappointing results, unless however you plan on gaining weight after surgery. Fat grafting over a hard bony surface such as the tailbone in general has very poor survival. Because of all this, you indeed may want to more strongly consider buttock implants to give you more volume in your butt cheeks which will potential avoid tue direct pressure over the tailbone.
