Pigmentation changes (color changes) can be caused by hormones (pregnancy--though I've never seen it in the earlobe) as well as repetitive trauma. The best place for you to start might actually be with a dermatologist or allergist/immunologist. Also, though it might be a challenge for you, the simplest thing may be to stop twisting your earlobes, if possible. Perhaps healing will occur on its own.
