A septoplasty is usually performed via incisions inside your nose. It can be done "open" if needed in severe cases. The septum would be completely removed from the nose only in very severe cases. This is called an extra-corporeal septoplasy. The septum is modified outside the nose, then replaced and the nose rebuilt around it.
A septoplasty is performed through closed approach through one incision approximately an inch long on the inside of the nose. Only the deviated component is removed( Bone and/or cartilage) leaving enough structural integrity to the nose.
It would be unusual to do an open septoplasty. An incision is made on the septum itself or though a transfixion incision, separating columella, and attached membranous septum, from the caudal cartilaginous septum.
The septum, the partition inside the nose, is operated on usually for airway improvement. Your nose is crooked and can only be corrected with an approporiate rhinoplasty. Both can be done at the same time.
