It's hard to say who will take responsibility. I would start by talking to your plastic surgeon. Implant rupture is a defined risk with breast biopsy, unfortunately, which they should have talked to you about before the biopsy.
Since there is always a risk to the implant with any breast procedure, this should have been discussed during the consultation. The biopsy probably was done to make sure you do not have a malignancy. If the biopsy was done by a general surgeon, he/she may not be comfortable with replacing the implant. If the biopsy was done by a radiologist, he/ she is not a surgeon and does not perform replacements. You will need to discuss this with your doctor who did the biopsy. Your insurance company will probably not assume any responsibility if you had a previous elective cosmetic augmentation.
