Patients with large chubby cheeks may have different results after buccal fat removal depending on two main factors: body weight and amount of buccal fat pad removal.Patients with normal or low normal weight will see a dramatic result after buccal fat removal. The higher the body weight, the less results will be seeing ( because the surrounding facial fat masks the results).The amount of buccal fat also is a main factors. if the cheeks are very chubby and very small amount of buccal fat pad is revoked, then the results will be less dramatic if all the buccal fat is removed 

Thank you for your question as well as your photos. Results for buccal fat removal are typically not visible for quite some time after surgery. It is an area that I would say takes the longest to get rid of swelling, similar to the nose and cheeks after a mid-facelift. I would speak to a Board Certified Plastic Surgeon to discuss filler that could potentially be injected into the zygoma giving a high cheek bone appearance to off-set the round look you're discussing.
