I am 28 years old and for as long as I can remember, I have always had a wide smile that cause my cheeks to go beyond my jawline and even make my eyes appear half open when I smile. I work out 3-4 days a week and have a healthy diet (I even have a slightly visible 6 pack) but for some reason and I can’t seem to lose weight in my face or have it have a slimmer appearance. When I am not smiling or talking, my appearance is fine but when i avoid smiling due to it feeling unflattering. What procedure can assist me with trimming down my face and achieving a more chiseled appearance?

Just based off your limited photo - its hard to gauge the cheeks and chin due to the beard and limited angle - but to answer your question - yes, deeper fat and / or visceral fat does increase as you gain weight post liposuction. There are a few factors for you - 1. is age, metabolism is slowing...
Based on these photos, profiles only, it appears that you would benefit from liposuction of neck/jawline and possibly chin augmentation with implant. It is impossible to discuss issue with cheeks without good frontal views. Hope this helps. Good luck.
