Thank you for sharing your question and photograph and I am sorry to hear of your persistent discomfort.  Most likely the pain and tenderness you feel is the result of the scar tissue from surgery.Because scar maturation can take upwards of a year to fully resolve I would talk to your surgeon about options to improve its quality, whether through silicone scar creams, sheets, massage, etc.  Hope this helps.
