Question

I’m 19 years old and have always had chubby cheeks and a round face even though I am skinny. I also have TMJ issues and my masseter muscle feels very stiff. I considered botox but I don’t think the results would be drastic enough. I was wondering if buccal fat removal would work along with chin filler? I am not concerned about aging problems because my face is still pretty round and would look youthful. When my face is resting it looks very heavy, but when I smile it turns into a V shape.

Young patients in their early teens who are very skinny yet have chubby cheeks or a round face typically have enlarged buccal fat pads. These fat pockets are usually genetically determined and resistant to diet and exercise, which is why it's common for slim individuals to have fuller cheeks. Buccal fat removal (bichectomía) is a plastic surgery procedure that permanently removes the fullness of the lower anterior face, resulting in a more slender, V-shaped facial appearance. The outcome depends on the amount of buccal fat removed. For those seeking a significant change, 100% removal of the buccal fat pad is recommended. However, candidates for this procedure must meet certain criteria.

In addition to having large cheeks, high cheekbones are a crucial requirement for buccal fat removal candidates. If high cheekbones are absent, they should be enhanced either before or during the procedure, usually with cheekbone fillers. This combination is likely to result in a V-shaped facial appearance and a reduction in the chubby smile for suitable candidates.

From the photos you would definitely benefit from a buccal fat removal and possible adjunct procedures such as chin augmentation (filler or implant) and possibly jawline contouring with neck lipo. Hard to tell from the photos. See someone virtually or in person for a consult! We do these a lot!!!
