I got a lower bleph 1.5 years ago and still have small bags. They've looked like this ever since the swelling went down. I have also had a CO2 fractional laser treatment under my eyes to tighten up the skin but I don't see any difference. Any suggestions on where to go from here?

Based on your photo, there appears to be some residual fullness under the eyes following your lower blepharoplasty. In such cases, a lower blepharoplasty revision may be considered to address the remaining under-eye bags.
This can often be performed using a transconjunctival approach, where the incision is made inside the eyelid, leaving no visible external scar. I hope this helps.

The lower eyelid can be challening to address as it first requires a good understanding of the exact cause of the fullness. In general this can be due to too much (or too little) fat, excess skin, or edema/swelling. A lower blepharoplasty can address the fat and sometimes excess skin. The laser is particularly helpful when the skin is the issue. My suspicion based on the photo is this may be edema/swelling which can commonly arise from trauma (from surgery or laser), allergies, or poor quality of sleep. Another option to consider would be a steroid/5-FU injection to see if this helps with swelling. If not, it may be worth while to look further into other causes of eyelid swelling like allergies, poor sleep, vitamin deficiencies, etc.

From the photo provided, there appears to be some degree of volume deficit in the upper outer cheek regions. If this is indeed so, then the use of a robust HA volumizing filler for cheek augmentation would not only create a more ideal feminine Ogee Curve, but would likely secondarily improve the under-eye region and diminish the residual bagging that disturbs you. In the hands of an experienced, board certified dermatologist filler injection expert, the treatment should take no more than ten minutes, yield immediate results and engenders little to no significant post-treatment downtime.
