7 months post op - non absorbable sutures used and now experiencing pain in scar area. What can I do to improve this? (photo)

I am almost 7 months post op, NON absorbable sutures were used so this isn’t a spitting stitch. I showed my surgeon and I was told all is normal , but it HURTS and it does not look like the rest of the scar. I was instructed to continue using silicone sheeting and massage, but it still does not show improvement … it appears worse. It looks like a blood blister that just won’t go away. Any advice is appreciated im just very lost on what this is and what to do. Thank you
