I had a breast reduction and lift many years ago , and several years later I decided to do silicone implants due to weight loss. I had a baby after that, but I feel like my breasts really changed. In fact my nipple are really embarrassing, they are different in placement. One is much higher and shows through every shirt or bra I wear !

Thank you for your question. It may be necessary for you to undergo a revision breast lift procedure. Your surgeon may recommend changing the type or size of implants or even replacing them to address changes in breast shape and volume. Additionally, nipple repositioning may be advised if there is a noticeable and undesirable asymmetry in nipple placement. It's important to schedule a consultation with a board-certified surgeon experienced in breast reconstruction and surgery. During the consultation, be sure to discuss any previous breast reduction, lift, or implant procedures, as well as any changes you've noticed since then, especially after pregnancy. Your surgeon will thoroughly examine your breasts and evaluate the condition of your existing implants, breast symmetry, nipple position, and skin elasticity.

Your breasts have bottomed out, your nipples are pointing upward and the implants have descended inferiorly and laterally. Most likely you will need a revision. At that time, I recommend removing the implants and the inframammary scars need revision to take in the excess loose skin. Also, at the same time, you may want fat transfers in the upper portion of the breasts to create upper fullness without the use of implants.

Thank you for your pictures and questions.  You have several options all based on re-doing your mastopexy or breast lift to correct the nipple malposition.  Once you are to have the breast lift revision, you can either keep the same implant, remove the implant or change it to a different size.  Be sure to see a Board-Certified PS for your best results.

It is not uncommon for breasts to change with pregnancy and breast feeding.  Surgery should be deferred until after breast feeding and pregnancy weight stabilization.  
Bottoming out is when a large part of the breast tissue and/or the implant falls below the nipple leaving the nipples too high. Options include revision of the breast lift with lowering of the nipple with or without breast implant exchange and trying to improve asymmetry.

The distance from the areola to the inframmamary crease in too long causing the breasts to bottom out. Skin needs to be tightened in a the inframammary area which is pretty simple.  Lowering a nipple that is too high is very difficult

Your tissue has been stretched out and is not supporting the weight of your breasts and implants. The lift can be done. Once the lower breast is lifted, you will probably see that your nipples are in the correct place. for best long-term results, I would suggest removing the implants and going smaller so less weight is on your tissue.

Your pictures help a lot. I do think your breasts can be restored to a normal appearance, but it will likely require more than one operation.

First would be an explantation and a substantial reduction in the overall size of your breast to get a normal shaped breast, but much smaller with more appropriate nipple placement. Your right nipple may or may not be OK in its placement depending on the bottom of your breast crease. Side photographs would help and certainly an in person consultation.

Just realize that this is not an easy fix and You should seek out an experienced surgeon.

Based on your posted photos, it appears you will need stage breast revisions. Stage one would be an attempt to lower the higher nipple Ariola complex by inferior placement with full thickness skin graph for defect above same time do revision breast lift.
