Generally a transplant would be pretty close to 100% survival, but it almost never hits the perfect survival mark, and a surgeon definitely couldn't guarantee that it would be 100% either. Even if only 90% survive, that's still a lot of grafts and it'll probably look just fine once you see the final results after a full year.
This will depend greatly on the skill of your surgeon and his surgical team. In the right hands a 4,000 graft procedure should yield in the high 90% range.
If the grafts are good (meaning they are intact with bulbs attached and have been kept in cool holding solution and not dried out) and are planted well (placed gently into the sites with minimal trauma) you should expect the great majority of your grafts to grow, >90%. The factors that I mentioned above, if not done properly leads to less survival of grafts.
