I want to get my implants explanted and I have lost my implant card. The surgeon who did my surgery is deceased unfortunately. I know the brand-The brand is mentor , that's all I know. Is there a way to find this info out with name and social and surgeons name. That's all I have.

If you would like to have this information prior to surgery for other reasons you can contact the manufacturer. The surgeon may have registered the implants with the manufacturer after surgery.
