I have very little fat and the loose skin is above the belly button. There is not loose skin below. I also have a small vertical scar in the crease above the belly button from a laparoscopic gallbladder removal (2004). Could the plication occur through an incision using this old scar in the crease and then tightened above? I do not want a hip to hip scar. In fact I’d rather not do anything then end up with a hip to hip scar.

Answer: Tummy tuck options If the only skin the bothers you is above the belly button, you theoretically could consider a reverse abdominoplasty. That would avoid a hip to hip scar, but instead would have a scar running across the front of the chest under the breast crease. This is not commonly done, but it is an option. Otherwise, in order to complete a muscle plication, you are going to need the classic hip to hip scar to create enough access to tighten the abdomen muscles and remove enough skin to have a great result. There are some surgeons that describe using a laparoscope to complete the muscle plication, but I am not sure about the utility of such an approach.
